Mo-Kan Sheet Metal Workers Welfare Fund Wellness Program

Last year Mo-Kan launched its first ever Wellness Program. Get Moving Mo-Kan was a huge success in screening 259 people.  259 people earned $100.00 in HRA credit just by participating in a self-reported health screening. This year those people will have an opportunity to earn another $100.00 by meeting their goal and an additional $100.00 for screening this year.  Mo-Kan invites you to join these folks in this program and learn about what is new for this year.

I am happy to announce the Mo-Kan trustees have approved Get Moving Mo-Kan to start an exercise program.  In 2009, if you join a health club, Mo-Kan will share the membership cost. If you join or already belong to a health club, Mo-Kan will reimburse up to $40.00 per month per person or up to $80.00 per family.

Participants of the plan, who regularly visit their health club and attend 12 times a month, will be reimbursed 50% of the cost up to the maximum of $40.00 for a single membership.  If two members of your family attend 18 times a month, Mo-Kan will reimburse 100% up to the maximum of $80.00 per family.  Participation in a wellness screening is required to participate in the health club program. If you were screened in 2008, you can take advantage right away. If not, once you have completed your 2009 screening, you can receive reimbursement. This is a very exciting time and we hope this is just another step in building the program.

In 2008 Mo-Kan members and their spouse will be able to earn HRA credit through healthily living.

How?

  1. Participate in the Mo-Kan Wellness Program. You can earn $100.00 in HRA credit for participating in a wellness screening in the first quarter of 2008.

A Health Reimbursement Arrangement is a supplemental way to help you pay for out of pocket medical expenses that you are required to pay under the terms of the Mo-Kan Sheet Metal Workers Welfare Fund contract.

What is a Wellness Screening?

A Wellness Screening includes:

  • Cholesterol-HDL (Good CHL)
  • Cholesterol-LDL (Bad CHL)
  • Total Cholesterol
  • Glucose
  • Blood pressure
  • Body composition
  • Waist circumference
  • Individual Health/Lifestyle Risk Assessment
  • One-on-one consultation

When the testing is complete, a qualified health coach will review your test results, a seven page individualized report, with you. You will be able to set goals with the coach and start on your way to earning HRA credit. Attached you will find a form that will be used to track your progress.

Is it mandatory for me to be involved?

The Wellness Screening is completely voluntary. This program is set up to make you aware of your own health care data and reward you for taking care of yourself.

All of your information will be kept confidential.

Who can participate?

To begin the program we are going to do screening on all active members, pre 65 retiree’s and their spouses.

How can I earn more HRA credit?

You can earn more HRA credit by meeting goals that you set with the health coach. You will be put into a risk category. There are 12 health risks and 4 risk categories that we would like to focus on. If you set a goal that helps you reduce the risk category that you are in and you maintain that goal you can earn another $100.00 in HRA credit. So, in a one year period you can receive up to $200.00 in HRA credit when you move risk categories or meet a goal that your health coach sets for you. Next year you can receive the same amount for maintaining your goal and re-screening.

How do I use the HRA credit?

Simply send in a receipt for any IRS qualified medical expense and Mo-Kan will send a check directly to your home for full reimbursement up to the amount you have in your HRA account. You will find a list of qualified medical expenses in this packet.

This program can make a difference in your individual life and help control healthcare costs for future Sheet Metal Workers.
