What is an (HRA) Health Reimbursement Arrangement?
An HRA is a supplemental way to help you pay for out-of-pocket medical expenses you incur under the terms of the Mo-Kan Sheet Metal Workers Welfare Fund contract.

You will receive $100.00 in HRA credit for screening in 2012.  With this health screening, Mo-Kan is addressing 12 modifiable risk factors.  In 2011 members were able to choose three (3) goals to work on throughout the year.  Those who met their chosen goal(s) when screened in the 2012 session will be monetarily rewarded up to $150.00 in HRA funds.  For those members who have not screened before, you will be able to set three (3) goals at the 2012 health screening for consideration of monetary reward if those goals are met in the 2013 health screening session.

To gain access to your HRA credit you will need to complete a reimbursement form, attach proof of payment and either fax to Mo-Kan at 816-753-7252 or mail to Mo-Kan Sheet Metal Workers Welfare Fund, P.O. Box 300019, Kansas City, MO 64129.

Gym Membership Program

Individual Membership

Family Membership

8 times a month = 50% up to $40.00

12 times a month = 50% up to $80.00

12 times a month = 100% up to 40.00

18 times a month = 100% up to $80.00

Mo-Kan will only reimburse for the actual gym membership fee.  Any additional fees your gym may charge are not payable under this benefit.  If you are new to the program, you may start receiving your refund after you complete your 2012 health screening.  If you screened in 2011, you may start receiving your refund in January, as long as you are scheduled for a 2012 screening.
